Telehealth is not for emergencies. If you have chest pain, trouble breathing, thoughts of harming yourself or any other emergency, call 911 or go to the nearest emergency room.
1. What telehealth means
Telehealth is the delivery of healthcare using electronic communications instead of an in-person visit. At THINKR this may include online questionnaires, secure messaging, photos, lab results, phone calls and video visits ("asynchronous" or "synchronous" care, depending on your state and treatment).
2. How care works at THINKR
THINKR is a technology platform. Your care is provided by independent clinicians — physicians, nurse practitioners and physician assistants — licensed in the state where you are located at the time of each visit. Your clinician reviews your information, may ask follow-up questions or order labs, and decides whether treatment is right for you. Your clinician may also decide that telehealth is not appropriate and recommend in-person care.
You can ask for your clinician's name and credentials at any time.
3. Benefits
- Care from home, without travel or waiting rooms.
- Faster access to licensed clinicians and easier follow-up.
- Ongoing messaging with your care team between check-ins.
4. Risks and limits
- Your clinician cannot physically examine you, which may mean a condition is missed or more information is needed.
- Decisions depend on the information you give. Incomplete or inaccurate information can lead to wrong decisions or harm.
- Technology can fail, be delayed or have poor quality, which could delay care.
- Despite security safeguards, there is a small risk that electronic information could be intercepted or accessed without authorization.
- Responses to messages are not immediate. Messages are generally answered within one business day.
5. Prescriptions
A clinician will prescribe medication only if they decide it is medically appropriate. Prescriptions are not guaranteed. Medications such as GLP-1s and hormone therapies have known side effects, which your clinician will explain; you can ask questions before starting. Some medications may be compounded by licensed pharmacies; compounded medications are not FDA-approved. Testosterone is a controlled substance and is prescribed only as federal and state law allow, which may require a video visit or in-person evaluation.
6. Your responsibilities
- Give complete, honest and current information about your health, medications and allergies.
- Complete any labs, video visits or follow-ups your clinician requests.
- Take medication only as prescribed and report side effects promptly.
- Be physically located in a state where THINKR offers your program during each visit.
- Use a private setting and a secure device for visits and messages.
- Keep a primary care provider for your other health needs.
7. Privacy and records
Your health information is protected under HIPAA and applicable state law, and is handled as described in our Privacy Policy. Your clinician keeps a medical record of your care, and you can request a copy. With your permission, or where the law allows, information may be shared with your other providers, pharmacies and labs to coordinate your care. Video visits are not recorded without your consent.
8. Your rights
- You may refuse or stop telehealth care at any time without losing your right to future care elsewhere.
- You may ask for an in-person referral instead.
- You may ask questions about your treatment and get answers before you agree to it.
- You may withdraw this consent by emailing support@thinkrmd.com. Withdrawal does not affect care already provided.
9. Emergencies
THINKR does not provide urgent or emergency care. If you experience a severe reaction (such as swelling of the face or throat, severe stomach pain, fainting, or signs of a blood clot like leg pain with swelling or sudden shortness of breath), stop the medication and call 911 or go to the nearest emergency room. Tell your THINKR clinician afterward. If you are in emotional crisis, call or text 988.
10. State-specific notices
Some states require additional disclosures or specific forms of consent for telehealth. Where required, your clinician will provide those disclosures and obtain your consent in the required format, such as verbally during a video visit. Where state law gives you more rights than this consent, state law applies.
11. Your agreement
By completing an assessment, checking the consent box or receiving care through THINKR, you confirm that you have read and understood this Telehealth Consent, had the chance to ask questions, and agree to receive care by telehealth.
